Please be aware that neither Women's Health Alliance nor COEST Laboratories provide child care services. Your children must remain with you at all times. They should NOT be left unattended in the waiting room or hallways. If you are having a radiologic procedure - Mammogram or Bone Density exam - please be aware that a child CANNOT be in the room during the procedure due to unnecessary radiation exposure. If you bring children with you, you will be asked to reschedule your appointment.
About your Visit
Your health care is a partnership, and you play a key role in that partnership. It is important that you feel free to ask questions, and that you bring any symptoms you might have to our attention. In addition, we ask you to take an active part in leading a healthy lifestyle, including appropriate self –examination, exercise, and proper eating habits.
Insurance Information
If you are covered by a private insurance carrier, our staff will gladly submit an insurance claim to your carrier on your behalf. Private insurance plans vary and may cover anywhere from zero to 100% of your medical and surgical charges. With the vast array of plans and coverage, it is impossible for our staff to determine the extent of your benefits for the typical office visit. We will, however, work very closely with you to determine your eligibility and precertification for any surgery or in-office procedures.
Patient deductibles and co-payments are due at the time of service.
Women’s Health Alliance is a participant of Medicare and will file your claim to Medicare and any secondary insurance you may have.
Patients without insurance coverage will be responsible for all charges incurred. At the time of service, payment in full is expected and customary. Should you need financial assistance, we will work with you so that charges will not be a burden.

Financial Arrangements
In order to keep healthcare cost down, we ask for payment at the time of your visit. For your convenience we accept cash, check, Via, MasterCard and Discover. We also have in place automated Patient Payment Assurance system which will eliminate any statements o return payments on your part.
Fees
Fees for normal obstetric care include prenatal care, vaginal or caesarean section delivery, and a six week postnatal care following delivery. Extra charges will be assessed for lab results, ultrasound exams, non-stress test, fetal monitoring, high risk pregnancy, amniocentesis, and Pap smear or other tests or procedures that may be required.
Surgical fees include surgery, physician time to perform the procedure, and one follow-up office visit. There will be additional lab charges for any required pre-operative testing. You will receive a separate bill from the hospital or outpatient facility and other providers. Please mail your payment directly to the billing provider.We understand that sometimes circumstances make it difficult to make payment on a timely basis. If necessary, we can arrange a payment plan for OB and GYN services. Please contact our billing department at 317-814-4505 if an unusual financial problem arises.

We cannot accept e-mail request for medical record copies. Please place your request in writing with your signature. You may either mail the form to our office of fax it to 317-814-4545

Scheduling an Appointment

Referrals
If you have been referred to Women’s Health Alliance by another physician or have a family physician, please inform us. We will be able to keep your physician current on your diagnosis and condition.
Forms
These forms are provided for you to download to your printer, complete and bring with you to your next visit. If this your first visit with us, completion of these forms will decrease the check-in time. If you a returning patient, we request a new HIPAA form plus Medical History and Insurance authorization if the information has changed since your last visit.
